Accuracy of Records and Data.

Our members need to be ensure that any information being used in reviews is as complete an accurate as possible.

You may have good reason to consent for your medical notes to be released and we remind you that such records might not be complete or may contain errors so you need to know what such information or data contains.  We have reports that some medical data returned to IOD's from their forces' following the ICO involvement, even contain information relating to other people!

In the course of the review we stress the need to keep any oral information to be simple, accurate, factual and unembellished.

Since my last review, on each consultation with either GP or Specialist, I have stressed the need for completeness and accuracy and explained  about the IOD review procedure, how forces have acted and the likelihood of future PMAB and Administrative Courts involvement.
